29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold(s) trained by a person qualified in the subject matter to recognize those hazards associated with the type of scaffold(s) being used and to understand the procedure(s) to control or minimize those hazards:                                                                                                             (a)  American Contracting, LLC at 1000 W. Pitkin St., Fort Collins, CO 80521: On and before April 17, 2015, the employer did not ensure that an employee tasked with partial demolition work inside of a building was instructed in the recognition and avoidance of unsafe conditions associated with the removal of metal ductwork. An employee was engaged in manually cutting and removal of a section of suspended metal ductwork in the basement level of a college dormitory building. The employer observed the employee cutting down an approximate 15 foot long section of ductwork while standing on a 6 foot high mobile scaffold. The employer recognized the struck-by hazard to the employee on the scaffold but did not instruct the employee in the recognition and avoidance of this hazard.
