29 CFR 1910.23(a)(1): Every stairway floor opening shall be guarded by a standard railing constructed in accordance with paragraph (e) of this section. The railing shall be provided on all exposed sides (except at entrance to stairway). For infrequently used stairways where traffic across the opening prevents the use of fixed standard railing (as when located in aisle spaces, etc.), the guard shall consist of a hinged floor opening cover of standard strength and construction and removable standard railings on all exposed sides (except at entrance to stairway):  a) At the rig, the space between the main deck/platform and the platform of the recirculating pump had a floor opening that was not guarded.

Handrails were not provided on at least one side of enclosed stairways preferably on the right side descending:  a) At the rig, the stairway leading to the recirculating pump from the main platform did not have a standard railing.  b) At the rig, the main/front stairway had the handrails broken/detached and bent (right side descending).  c) At the rig, the rear stairway leading to the generator had the handrails broken/detached and bent (right side descending).

29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers was not provided, used, and/or maintained:  a) At the rig, fire-retardant and heat-resistant clothing was not provided to employees working near an unguarded engine manifold exhaust,  exposing the workers to burns.  b) At the rig, work gloves were not provided at no cost to employees performing maintenance tasks.  Workers were exposed to hazardous chemicals damaging to the skin such as, but not limited to, oils, lubricants and diesel petroleum.
