29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a. On or about May 4, 2015, roof area, employees removing metal panels were not protected from falling 9.6 feet to the ground by the use of a safety monitor and warning line system.

29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function:  a. On or about May 4, 2015, roof area, employees removing metal panels were not protected from falling 9.6 feet to the ground because the safety monitor had other responsibilities such as helping to remove the panels, cutting panels, providing materials as needed.  The safety monitor was changing a blade on a saw when the accident happened.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    a. On or about May 4, 2015, an employee used an 8-feet step ladder in the closed position leaned against the wall while using a hand held grinder to grind the iron beam.  This exposed the employee to fall hazards.
