29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested:   a)Paint booth and area:  On or about May 21st, 2015, and prior to, employees were exposed to inhalation hazards from chemicals in paints to include 1,6 Hexamethylene Diisocynate while conducting spray painting work tasks with required use of a half mask respirator and not provided fit test for the respirator.

29 CFR 1910.134(k)(3): Training was not provided prior to requiring employees to use a respirator in the workplace:   a)Robert Lucassen Paint and Body, Inc.:  On or about May 21st, 2015, and prior to, the employer exposed employees to inhalation hazards from dust and paint fume containing isocyanates while requiring use of tight fitting respirators and has not provided annual training or training the employees on Appendix D of the respirator standard when dust masks are used voluntarily.
