Neither was the equipment approved for the class of location and for the ignitable or combustible properties of the specific gas, vapor, dust or fiber that would be present:    (a) Throughout the Foster Custom Woodworks, Inc. facility - On or about May 21-27, 2015 employees engaged in cabinet making, i.e., machining combustible Medium Density Fiberboard (MDF), were exposed to fire and explosion hazards due to electrical equipment and wiring in the following areas not being dust-tight, ignition-proof (i.e., dust ignitionproof) and approved for Class II, Division 1 & 2, Group G combustible dust locations, in that general purpose wiring components and lighting were used throughout, e.g.:     (1) NEMA 1 general purpose duplex outlets and outlet boxes, without faceplates, were mounted on the walls and work tables;  (2) Conductor openings in junction boxes, disconnect panels, Phase-A-Matic Phase Converter, etc. were not sealed whether conductors were present or not;  (3) The Amerock Door Drilling (Hinge) Machine's junction box had no cover;  (4) Wiring to/from the electrical box on back side of the Jet 36 inch wide Belt Sander had unsealed openings, and was not approved for a Class II, Division 1 location;  (5) The fluorescent lighting fixtures in the Production Shop were not approved for a Class II, Division 1 or 2 locations.    (b) Throughout the Foster Custom Woodworks, Inc. Paint Room - On or about May 21-27, 2015 employees engaged in cabinetry finishing operations, where volatile flammable liquids were being handled and sprayed, were exposed to fire and explosion hazards due to the presence of the following general purpose wiring components and electrical equipment which were not explosionproof and approved for Class I, Division 1, Group D, locations in that:   (1) The plastic exhaust fan in the Paint Room window was not explosionproof, nor approved for Class I, Division 1 & 2 locations, and  (2) The fluorescent lighting fixtures and wall outlets in the Paint Room were not explosionproof , nor approved for Class I, Division 1 & 2 locations.
