29 CFR 1926.1053(b)(14): Cross-bracing, not designed for climbing, on the rear section of (a) stepladder(s) was used for climbing:    On or about May 20, 2015, at the job site located at 859 E Milsap in Fayetteville, Arkansas, employees on the roof, were using a 6 foot step ladder, to erect steel posts, for a screen wall. The employee on the ladder straddled the top of the ladder, and used the top cross brace as a step,  to stand on while driving the post. These conditions exposed the employee to the hazard of falling approximately 6 feet to the roof below.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    On or about May 20, 2015, at the job site located at 859 E Milsap in Fayetteville, Arkansas, employees who were required to work from the roof of the new structure, while erecting steel posts for a screen wall, were not utilizing any type of fall protection. These conditions exposed the employees to the hazard of falling from heights of approximately 16 - 18 feet to the ground below.      NWA Steel Company, LLC. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 908462, citation number 1, item number 1 and was affirmed as a final order on August 26, 2013, with respect to a workplace located at 5336 Central Avenue in Hot Springs, Arkansas.
