Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards created by working on an unsecured piece of a centrifuge compressor.     a)  Production Floor - On or about May 5, 2015, an employee was disassembling the compressor of a 50PDI Centrifuge when the Return Bend dislodged, fell to the ground, and struck the employee.     Abatement Note:  Among other methods, one feasible method of abatement to reduce these hazards is to provide and follow pertinent sections of the 50PDI Equipment Manual.  These sections include, but are not limited to, General Safety Precautions, Disassembly Procedures, and Reassembly Procedures.  Additionally, task specific guidance is included in the system specific instructions provided on Page 5-7.1 Item B5, wherein it states to rig the unit to a hoist prior to removing the capscrews, which attaches the return bend to the head.
