Attic storage area; on or about 10 June 2015 and times prior thereto; three propane cylinders were stored in front of the electrical panel in the garage bay area. This condition exposed employees to an electrical shock and electrical burn hazard.

Attic storage area; on or about 10 June 2015 and times prior thereto; Three incandescent light fixtures located 56 inches from the floor were not guarded. This condition exposed employees to an electrical shock and electrical burn hazard.

Garage bay, attic access; on or about 10 June 2015 and times prior thereto; A mid rail was not provided on a stair way with eight risers. Stairs are used to access a storage area in located the attic. This condition exposed employees to a fall hazard.
