29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):    a American Home Manufacturing LLC Piscataway NJ Site: Warehouse. The employer did not conduct a complete hazard assessment for personal protective equipment to ensure that required personal protective equipment had been selected and made available where needed, e.g., when handling boxes of product, handling wooden pallets, using manual box cutting tools, removing plastic shrink-wrap material from product, performing sorting operations and operating powered industrial trucks.    Observed on/or about 06/11/2015.

29 CFR 1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a/ American Home Manufacturing LLC Piscataway NJ Site: Warehouse. The eye wash was not in operating condition in that the handle used to release the water was broken.   The eye wash was not immediately accessible in that 15 minutes of continuous flow was not provided.    Observed on/or about 06/11/2015.

29 CFR 1910.178(l)(4)(iii):  An evaluation of each powered industrial truck operator's performance was not conducted at least once every three years:    a/ American Home Manufacturing LLC Piscataway NJ Site: Warehouse.  Permanent and temporary forklift truck operators had not been evaluated relative to the training they had previously been provided by the site employer and the temporary agency.  Temporary employees were not evaluated when hired.  Permanent employees had not been evaluated at least every 3 years.    Observed  on/or about 06/11/2015.
