29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   A) Pallet on lift - An employee was exposed to fall hazards of approximately 16 feet while working on a wooden pallet that was supported by a JLG lift. The employee was not provided with a personal fall arrest system or any other means of fall protection on or about March 19th, 2014.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.   A) Exterior, On lift/pallet: An employee was exposed to head injuries while working below window installation activities while not protected with any type of head protection, on or about July 20th, 2015.     Frame Q Custom Carpentry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.100(a), which was contained in OSHA inspection number 1042114, citation number 1, item number 1 and was affirmed as a final order on July 27, 2014, with respect to a workplace located at 368 Edgewood Avenue in Teaneck, NJ.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:                                                                                                             A) Upper Level of structure in window: An employee was exposed to eye/face injuries while using a pneumatic nail gun and was not protected with any type of eye/face protection, on or about June 20th, 2015.   B) Upper Level of structure inside: An employee was exposed to eye/face injuries while using an electric saw and was not protected with any type of eye/face protection, on or about June 20th, 2015.   Frame Q Custom Carpentry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1042114, citation number 1, item number 1 and was affirmed as a final order on July 23, 2015, with respect to a workplace located at 27 Maple Avenue in Mahwah, NJ.
