29 CFR 1910.178(l)(2)(ii): Training did not include an evaluation of the operator's performance in the workplace:  a- Northside Imports, Inc., Edison, NJ - An employee, who was trained in forklift operations through a previous employer, was not evaluated by the current employer, to ensure the effectiveness of the training received.  The employer must also certify that each operator was trained and evaluated.  The certification is to include, but not limited to, the name of the operator, the date of the training or evaluation and the name of the person providing the training or evaluation.  Condition noted on or about 6/18/15.
