On or about June 3, 2015, the employee operating the Ryobi saw to cut wood did not have on safety glasses and being exposed to flying debris.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:    a. On or about June 3, 2015, front entrance,  employees climbed the scaffold rungs and crossbraces because there was no ladder.  Employees were exposed to fall hazards up to 12.5 feet when accessing the top level.

On or about June 3, 2015, front entrance to the building, the two-buck high scaffold system did not have guardrails and end rails and employees were being exposed to fall hazards up to 12.5 feet high.
