29 CFR 1926.1060(b):     The employer did not provide retraining as necessary so that each employee exposed to fall hazards maintained the understanding and knowledge acquired through initial training:    a)  Traditions of America - Saucon Valley - On or about June 25, 2015, an extension ladder used by sub-contractor employees did not extend at least three feet above the upper landing surface.
