29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards:  (a)  Employee(s) accessing the Blend 3 mixer tank hatch from a ladder platform in the Powder Blending & Packing department were exposed to a fall hazard of approximately 94 inches to the floor due to no fall protection system being used.  (b)  Employee(s) accessing the Blend 2 mixer tank opening from a ladder walking platform in the Powder Blending & Packing department were exposed to a fall hazard of approximately 76 inches to the floor due to no mid-rail being provided for the fixed ladder access way.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to 29 CFR 1910.147(c)(4)(ii)(B), (c)(4)(ii)(C), and (c)(4)(ii)(D):  Throughout the facility, the specific energy control lockout / tagout procedures for production machinery or equipment did not clearly and specifically include the following:  (a)  Specific procedural steps for shutting down, isolating, blocking and securing equipment of hazardous energy.  (b)  Specific procedural steps for the placement, removal and transfer of lockout or tagout devices, and who is responsible for the devices.  (c)  Specific requirements for testing equipment or machinery to verify the effectiveness of the lockout / tagout devices.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  The employer failed to provide lockout / tagout training for all authorized line leaders, formulators, and operators in recognizing hazardous energy, the methods, and means necessary for isolating the production equipment, thus exposing employee(s) to amputation or struck by hazards.
