OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish the employees employment and a place of employment which were free from hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing/caught-in-between hazards:    a) Jobsite located at 151 Benigno Boulevard, Bellmawr NJ:  On or about June 10, 2015, the employer failed to ensure employees were protected from crushing hazards associated with unexpected flatbed/trailer movement while loading machinery including but not limited to: a SAKAI vibrating roller SW320 4000 VPM and a Caterpillar Tandem Vibratory Roller Model #CB534D without using chocks on the flatbed trailer's wheels.      Abatement method:  Instructions included in the SAKAI Model # SW320 Vibrating Roller operators manual on loading and unloading state "Engage the trailer brake and chock its wheels" (Page 39) A diagram on the same page indicates where the chock blocks should be placed.
