29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: (a) Front side, new residential construction located at 1561 Whitfield Street, Smyrna, GA 30080: Employee was exposed to falls of approximately 14-feet, 4-inches to the lower level while working from a job made scaffold platform which was only 5-1/2-inches wide. The employee engaged in framing activities was not provided with a scaffold platform which was at least 18-inches wide.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: (a) Back side, new residential construction located at 1561 Whitfield Street, Smyrna, GA 30080: Employees were exposed to fall hazards of approximately 10-feet, 9-inches while accessing the lower level from the second floor as the ladder side rails were not extended past the landing surface or secured to the structure to allow employees safe access to the lower level.
