29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly, and in a sanitary condition.    On or about July 14, 2015, and times prior thereto, employees were required to work where the following hazards existed throughout the facility:    a) Walls, floor tiles, and ceiling tiles were moldy and deteriorated due to damp conditions caused by water leaks and standing water, thus creating significant musty/moldy odors which caused employees to suffer from headaches and/or respiratory-related illnesses.   b) Moldy, deteriorated ceiling tiles were dangling from ceilings and/or piled on floors, exposing employees to struck-by and/or tripping hazards.  c) Floor surfaces where floor tiles had rotted and broken apart exposed employees to tripping hazards.  d) Telephone cables, data cables, and other equipment cords were strung overhead and strung across aisle ways, exposing employees to tripping and/or caught-in hazards.

On or about July 14, 2015, and times prior thereto, employees were required to walk through muddy, wet floors to access the door located at the back of the facility to accept deliveries and/or accessed for the emergency exit route.  This exposed the employees to slipping/tripping hazards.

On or about July 14, 2015, and times prior thereto, an elevator pit located in the area where employees' desks were located and where employees worked was covered by unstable flooring.  This exposed employees to a fall from a height greater than six (6) feet to the basement floor.
