29 CFR 1910.23(a)(2): Every ladderway floor opening or platform must be guarded by a standard railing with standard toeboard on all exposed sides (except at entrance to opening), with the passage through the railing either provided with a swinging gate or so offset that a person cannot walk directly into the opening.  (a) On or about July 20, 2015, the employer failed to provide guarding for ladderway floor openings in work platforms.      (1) Vecoplan Shredder: The ladderway floor opening in the work platform was unguarded. Employees were exposed to fall hazards.      (2) Macpresse Baler: The ladderway floor opening in the work platform was unguarded. Employees were exposed to fall hazards.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level must be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder.   (a) On or about July 20, 2015, the employer failed to provide work platforms 4 feet or more above adjacent ground level with standard railings.          (1) Vecoplan Shredder: The employer failed to provide a midrail on the railing around the work platform. Employees were exposed to fall hazards.      (2) Macpresse Baler: The employer failed to provide a midrail on the railing around the work platform. Employees were exposed to fall hazards.
