29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers:  a) Trailer Platform - Ankiewicz Enterprises Inc. employees retrieving and putting supplies in the trailer were exposed to a slip, trip and fall hazard from a ten (10) inch gap between the deck and the trailer, on or about July 22nd, 2015.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
