29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:  On or about July 22, 2015, at the above addressed jobsite, 16300 NW 49th Avenue, Miami Gardens, Florida 33014, employees are offered U-Line N-95 respirators for respiratory protection from metal dust. Without the employees being given Appendix D of CRF 1910.134, the employees do not know there are limits of the respirator, how to clean and care for the respirator, and what type of contaminants the respirator offers protection.
