29 CFR 1910.147(c)(5)(ii)(A)(1): Lockout and tagout devices were not durable; they were not capable of withstanding the environment to which they were exposed for the maximum period of time that exposure was expected:    On or about July 20, 2015, at the job site near Highway 77 and Rucker Road, Junction City, Kansas, a Maintenance Technician did not use a durable lockout device to ensure the raised cab/seat on the 289D CAT Skid Steer Loader would not come down when maintenance was performed.  The employee was exposed to struck-by/caught between hazards.
