29 CFR 1910.24(b): Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations: Wisconsin Building Supply truss plant located at 1990 Larson Rd. Green Bay, WI: On and before 7/22/15, employees required to climb 3' from the floor level up and onto the truss assembly table were not provided with a fixed or portable stair to assist them in the transition from one level to the other.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Wisconsin Building Products truss plant at 1990 Larson Rd, Green Bay, WI: On and before 7/22/15, the MITEK Truss Glider press was not effectively guarded to prevent employees from inadvertently placing parts of their body into parts of the machine that could cause amputations and/or crushing injuries.
