29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:      On or about, July 18, 2015, employees were exposed to fall hazards greater than 13 feet when climbing into a grain bin with the use of a collapsed "A" frame ladder that was leaning against the grain bin. The ladder was not used as designed, in an open position, to prevent tipping and/or kick-out of the ladder.
