29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) Chase Roofing LLC, Defense Logistics Agency Building SDA-204: On or about 08/06/2015, the path to ground on the 100 foot orange extension cord was not permanent and continuous in that the ground prong on the male attachment plug was missing thus exposing employees to a possible shock injury. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) Chase Roofing LLC, Defense Logistics Agency Building SDA-204: On or about 08/06/2015, employees working on a flat roof approximately 20 feet wide were not provided with fall protection while disconnected from the horizontal lifeline, as a result an employee fell 15 feet to the ground. No abatement certification or documentation required.

Some area along the rope lines were bird caged, frayed and had broken stitching. No abatement certification or documentation required.
