The exposed employee was directed to enter and exit the trench without the use of a ladder. The work being done was the installation of one inch water line. This process consisted of earth removal, trench floor leveling, boring/mole process, water line installation, water line fitting set up, water connection, and back fill. The process exposed employees inside the trench for approximately 45 to 90 minutes per trench. The exposed employees were working in the trench while being directly supervised by the site foreman Mr. Andy Willemsen. The side walls of the trench were near vertical.  The employer classified the soil as type C. There was no shield system, benching, and/or sloping of the side walls to adequately protecting the laborer from a cave-in, while he was working inside the trench.    b) On or about August 12, 2015, at a job-site located at 3444 Adaline Dr, Stow, Ohio 44224; the employer did not ensure a crew consisting of a foreman and three laborers had means of access and or egress while installing one inch water line in a trench with a depth of 6 feet.
