29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Robot area, On or about August 12, 2015, employees were exposed to struck-by hazards from the unexpected reenergization of an automated robot arm while entering the gated working area to make bag adjustments without specific lock out tag out procedures and training in place.

29 CFR 1910.272(j)(2)(ii): The employer did not immediately remove any fugitive grain dust accumulations whenever the exceeded 1/8 inch (.32cm) at priority housekeeping areas, pursuant to the housekeeping program for grain elevators: Scratch plant area, on or about August 12, 2015, employees were exposed to explosion, deflagration and /or fire hazards as a result of the accumulation of combustible dust on horizontal and vertical surfaces was greater than 1/8 inch in thickness. The Allied Feed was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.272(j)(2)(ii), which was contained in OSHA inspection number 314307521, citation number 1, item number 4 and was affirmed as a final order on August 11, 2011, with respect to a workplace located at 4542 Rigsby Ave. San Antonio, TX. 78222.
