29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye when exposed to eye or face hazards from liquid chemicals, acids and/or caustic liquids:    a) Kitchen:  Employees handled and used various chemical cleaners including but not limited to:  Greasestrip (sodium hydroxide), Lime Away (uronium hydrogen sulfate), (Dip-It Xp (sodium metasilicate, sodium carbonate peroxhydrate), etc. and were not required to use appropriate eye protection (e.g., chemical splash goggles) on or about 8/18/15.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:      a) Kitchen:  Employees moved heated carts (approximately 160 deg. F) with hot surfaces by hand from the kitchen to pantries on the 1st and 2nd floors and the employer did not select and require employees to use appropriate hand protection (e.g., heat resistant gloves or equivalent); on or about 8/18/15        b) 1st and 2nd Floor Pantries:   Employees took hot metal pans from heated carts (approximately 160 deg. F) and the employer did not select and require employees to use appropriate hand protection (e.g., heat resistant gloves or equivalent); on or about 8/18/15.    c) Kitchen:  Employees handled and used various chemical cleaners including but not limited to:  Greasestrip (sodium hydroxide), Lime Away (uronium hydrogen sulfate), (Dip-It Xp (sodium metasilicate, sodium carbonate peroxhydrate), etc. and the employer did not select and require employees to use appropriate hand protection (e.g., chemical resistant gloves); on or about 8/18/15.
