29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 888 HIGHLAND STREET, STEELTON, PA - Employees performed roofing activities, and the employees were observed not using any type of fall protection, thereby exposing the employees to fall hazards that ranged from approximately eight (8) to twenty-five (25) feet to the ground below, on or about August 24, 2015. No abatement certification or documentation required.

29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection: 888 HIGHLAND STREET, STEELTON, PA - Employees performed roofing work, and the employees were not provided with adequate fall protection equipment to safely complete the job, and were not trained on the correct usage of the fall protection that was available, thereby exposing the employees to possible fall hazards, on or about August 24, 2015. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) 888 HIGHLAND STREET, STEELTON, PA - Employees performing roofing work accessed the roof by using a 28 foot extension ladder, and the rungs of the ladder only extended approximately thirty (30) inches past the upper landing surface, thereby exposing the employees to possible fall hazards, on or about August 24, 2015. No abatement certification or documentation required.
