29 CFR 1928.57(a)(6): The employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he was or would be involved at the time of initial assignment and at least annually thereafter:  a) For employees involve in the sod planting operations at 8780 NW Bethel Farms Road, Arcadia (FL), on or about 9/2/2015.  In that employees working with power take off shafts and farm field equipment such as but not limited to sod planter and tractor:  1. were not instructed to keep all guards in place when the machine is still in operation; and   2. were not instructed about the procedures to service, adjust, clean or unclog equipment such as but not limited to stop engine, disconnect the power source, and wait for all machine movement to stop.
