29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:   a) Located at Simco Logistics Inc. DBA Jack & Jill Ice Cream Co, 501 Commodore Drive, Swedesboro NJ: On or about 8/22/15, an employee received an injury to his skull resulting in a hospitalization. The employer reported the hospitalization to OSHA on 8/25/2015, not within the required 24 hours.
