29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   On or about 9/2/2015 at 486 Cabot Trace, Grayson, GA; a worker was observed carrying materials from the elevator to the rear of the roof in support of felt and shingle (new) installation on a 10/12 pitch roof, exposed to fall hazards of 25' at the peak and 19' 6" at the eve where he was observed not connected to the roof.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  On or about 9/2/2015 at 486 Cabot Trace, Grayson, GA; two workers were observed performing felt and shingle (new) installation on a 10/12 pitch roof, exposed to fall hazards of 25' at the peak and 19' 6" at the eve where the workers were observed tied off to their fronts and not properly connected to the D rings on their full body harnesses, to the centers of their backs .
