29 CFR 1910.24(h): Standard railings were not provided on the open side(s) of all exposed stairways and platforms. Stair railings were not installed in accordance with the provisions of Sec. 1910.23. On the inside of the CM19 Compressor building, stairways less than 44 inches wide that employees used to access the working platforms of at least two compressors did not have standard railings on all opened sides, exposing employees to a fall hazard up to approximately 7 feet to the ground below.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use. Outside the CM19 compressor building, unused helium cylinders were stored without the protective caps exposing employees to struck-by hazards.

29 CFR 1910.1200(f)(6): The employer used written materials, such as signs, placards, process sheets, or batch tickets in lieu of affixing labels to individual stationary process containers that failed to identify the container(s) to which they were applicable and did not convey the information required by 29 CFR 1910.1200(f)(5). Outside the CM19 compressor building, the employer did not ensure that containers with chemicals were labeled or marked with product information exposing employees to inhalation or dermal hazards.
