29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:     (a) Jobsite, Roof:  On or about September 14, 2015, an employee was working from a mobile (Bakers) scaffold and none of the wheels were locked.  The fall hazard to the employee was about 6 1/2 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:     (a) Jobsite, Roof:  On or about September 14, 2015, an employee was not protected from falling while he was working near the edge of the roof.  The fall hazard to the employee was about 55 feet to the ground.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employees knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:   (a) Jobsite:  On or about September 14, 2015, at least three employees need to be retrained on when they need to use fall protection and how to properly use fall protection.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system:      (a) Jobsite:  On or about September 14, 2015, an employee working in an aerial life approximately 60 feet above the ground was not tied off to the boom nor the basket.      The DMI, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 998729, citation number 1, item number 1 and was affirmed as a final order on 12/6/2015, with respect to a workplace located at 112 Commonwealth Drive, Lemont Furnace, PA.
