29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  On or about September 16, 2015:  Employees in the corrugated metals area (upstairs) used corrosive International Chemical 262-15 forming liquid while manufacturing hoses.  The employer provided two bottles of eyewash liquid, which was not a suitable facility for flushing of the eyes.

29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information specified under paragraphs (f)(6)(i) through (ii) of this section:  On or about September 16, 2015:  Employees in the corrugated metals area (upstairs) used corrosive International Chemical 262-15 forming liquid while manufacturing hoses.  The employer failed to ensure all containers of chemicals were labeled with its contents and warnings.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  On or about September 1, 2015:  Employees in the stiller area were exposed to a chemical release due to the degreaser overheating and the acidification of perchloroethylene.  The employer failed to ensure employees received hazard communication training prior to the use of the chemicals.
