29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as used as a means of access. a. Located at 755 West Waterloo in Canal Winchester, Ohio: On or about September 25, 2015, employees were using the scaffold frame to access the working platform of a Scaffolding Today Inc. fabricated frame scaffold. The scaffold frame was not an approved access, thereby exposing employees to a maximum fall hazard of approximately eighteen foot.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a. Located at 755 West Waterloo in Canal Winchester, Ohio: On or about September 25, 2015, employees were performing masonry work from a Scaffolding Today Inc. fabricated frame scaffold without adequate guard rails on the ends of the side bracket platform, exposing employees to an approximately eighteen foot fall hazard, and on the open sides on part of the main scaffold platform, exposing employees to an approximately thirteen foot fall hazard. Silver Rule Masonry Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4)(i), which was contained in OSHA inspection number 1051340, citation number 1, item number 4 and was affirmed as a final order on May 27, 2015, with respect to a workplace located at 289 Naghten Street, Columbus, Ohio.

29 CFR 1926.451(h)(1): In addition to wearing hardhats each employee on a scaffold shall be provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. When the falling objects are too large, heavy or massive to be contained or deflected by any of the above-listed measures, the employer shall place such potential falling objects away from the edge of the surface from which they could fall and shall secure those materials as necessary to prevent their falling. a. Located at 755 West Waterloo in Canal Winchester, Ohio: On or about September 25, 2015, employees were performing masonry work from a Scaffolding Today Inc. fabricated frame scaffold. Employees working below the scaffold were not protected from falling tools and materials by means of, but not limited to, toeboards. Silver Rule Masonry Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(h)(2)(ii), which was contained in OSHA inspection number 1051340, citation number 1, item number 5 and was affirmed as a final order on May 27, 2015, with respect to a workplace located at 289 Naghten Street, Columbus, Ohio.
