Toledo, OH: The employer failed to ensure employees were protected from falls through fiberglass skylight panels while performing roofing activities atop a low sloped commercial structure. Employees applying a tar sealant to the structures steel roof were exposed to an approximate 13 ft. fall through the uncovered skylights to the interior concrete floor below.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) Tri-Dim Filter Corporation on or about September 30, 2015, at the address of 1045 Matzinger Rd. Toledo, OH: The employer failed to ensure employees were protected from falls while performing roofing activities atop a low sloped commercial structure. Employees applying a tar sealant to the structures steel roof were exposed to an approximate 16 ft. fall from the eave to the ground below.
