29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    a) Tri-Dim Filter Corporation on or about September 30, 2015, at the address of 1045 Matzinger Rd. Toledo, OH: The employer failed to ensure employees were protected from falls through fiberglass skylight panels while performing roofing activities atop a low sloped commercial structure. Employees applying a tar sealant to the structures steel roof were exposed to an approximate 13 ft. fall through the uncovered skylights to the interior concrete floor below.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a) Tri-Dim Filter Corporation on or about September 30, 2015, at the address of 1045 Matzinger Rd. Toledo, OH: The employer failed to ensure employees were protected from falls while performing roofing activities atop a low sloped commercial structure. Employees applying a tar sealant to the structures steel roof were exposed to an approximate 16 ft. fall from the eave to the ground below.

Toledo, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing low sloped roofing operations.
