29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Building A2, On or about October 02, 2015 and at times prior therefore, employees were exposed to falls of approximately 10 feet to the ground level below, when not trained in the recognition of fall hazards, while on the balcony with unsecured guard rails.

29 CFR 1904.39(a)(2): 29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on the OSHA public web site at www.osha.gov:      On or about September 21, 2015, the employer failed to report to OSHA a work-related death/in-patient hospitalization/amputation/loss of an eye of one or two employees, when the hospitalization occurred on September 21, 2015 and was not reported to OSHA until September 30, 2015.
