29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    On the roof of a commercial building, north side, on or about October 15, 2015, an employee without fall protection was exposed to fall hazards greater than 6 feet to the paved ground below, while replacing roofing shingles on a roof with a pitch of 3:12.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On the roof of a commercial building, west side, on or about October 15, 2015, an employee without fall protection was exposed to a fall hazards of 18 feet to the paved ground below, while not being trained on the procedures to recognize fall hazards.
