29 CFR 1910.106(d)(7)(i)(b): A portable fire extinguisher having a rating of not less than 12-B units was not located not less than 10 feet, nor more than 25 feet, from any Category 1, 2, or 3 flammable liquid storage area located outside of a storage room but inside a building as required:   A) By slitter machine, on or about October 15, 2015, the employer did not have a portable fire extinguisher located between ten feet and twenty five feet of flammable storage cabinets.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  A) Facility, on or about October 15, 2015, the employer did not perform a periodic review of the lockout/tag out program for equipment such as the slitter machine.
