The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  29 CFR 1910.147(c)(4)(ii)(A): A specific statement of the intended use of the procedure;  29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;  29 CFR 1910.147(c)(4)(ii)(C): Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and  29 CFR 1910.147(c)(4)(ii)(D): Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  On or about October 8, 2015, Hirschvogel, Inc., South Third Street, Columbus, Ohio establishment: For employees performing required duties covered by this part, an energy control procedure for the "Lube pump" that services press AMP-11 with the specific statement of use, specific procedural steps and specific requirements for testing required by 29 CFR 1910.147(c)(4)(ii)(A)-(D) had not been developed, documented and enforced. On October 6, 2015, an employee performing troubleshooting duties on "Lube pump" equipment associated with AMP-11 suffered a partial finger amputation when the energized equipment cycled/was actuated downward.

The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  29 CFR 1910.147(d)(4): Lockout or tagout device application.  29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.  On or about October 6, 2015, Hirschvogel, Inc., South Third Street, Columbus, Ohio establishment: For employees performing required duties covered by this part, lockout/tagout devices were not applied to each energy isolation device, such as the ball valve which controls pneumatic energy for the "Lube Pump" associated with AMP-11. While performing troubleshooting duties on this equipment, an employee suffered a partial finger amputation when the energized equipment cycled/was actuated downward.
