29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems: (a) Best Team Construction, at 1167 South Garfield Street, Denver, CO 80210: On and before October 23, 2015, the exposing employer failed to ensure that employees engaged in residential construction activities 6 feet or more above lower levels were protected by guardrail systems, a safety net system, or a personal fall arrest system. Condition exposed employees to fall hazards of approximately 20 feet and greater as they performed roofing work.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee attached: (a) Best Team Construction, at 1167 South Garfield Street, Denver, CO 80210: On and before October 23, 2015, the exposing employer failed to ensure that anchorages used for attachment of personal fall arrest equipment were capable of supporting at least 5000 lbs per employee attached. Specifically, two employees were observed using one anchorage point, which due to its location and position, could not be verified to support at least 5000 lbs per employee attached (or 10000 lbs for two employees).
