29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:     a.  On or about October 21, 2015, two employees working on a 5/12 pitch roof installing asphalt shingles, were both tied into an anchor point not designed for two employees and were not supervised by a qualified person, exposing the employees to a 8 foot 6 inch fall hazard.    b.  On or about November 16, 2015, two employees working on a 5/12 pitch roof installing asphalt shingles, were both tied into an anchor point not designed for two employees and were not supervised by a qualified person, exposing the employees to a 11 foot fall hazard.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:     a.  On or about October 21, 2015, the employer did not ensure employees working on a 5/12 pitch roof while installing asphalt shingles, were trained by a competent in the correct procedures in using personal fall protection equipment, exposing the employees to a 8 foot 6 inch fall hazard.    b.  On November 16, 2015, the employer did not ensure employees working on a 5/12 pitch roof while installing asphalt shingles, were trained by a competent in the correct procedures in using personal fall protection equipment, exposing the employees to a 11 foot fall hazard.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    a.  On or about October 21, 2015, an employee working on a 5/12 pitch roof while installing asphalt shingles fell 9 feet 6 inches from the roof to the lower level sustaining injuries with his lifeline attached to his harness.     Holloway Roofing unlimited, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 472548, citation number 01, item number 02 and was affirmed as a final order on August 15, 2012, with respect to a workplace located at 422 Bally Way, Niceville, FL 32578.
