OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to amputation and laceration hazards.       255 Brooks St, Worcester, MA.      On 10/22/15 and at times prior, employees as a standard practice used the rotating action of pipe threading machines to add fittings to lengths of pipe.  On 10/9/15, an employee received a left index fingertip amputation when attempting to use a pipe wrench to add an elbow fitting to a pipe while the pipe was rotating in a Ridgid 300 Pipe Threader machine.  Among other methods, one feasible and acceptable means of abatement includes but is not limited to: comply with the manufacturer's operator's manual which contains specific safety information which prohibits making or breaking fittings while operating the pipe threader.
