29 CFR 1926.1053(b)(4):  Ladder(s) were used for purposes other than the purpose for which they were designed:    a) On or about October 28, 2015, the employees were exposed to fall hazards when two extension ladders were lashed together over the gate of a coffer dam to enable the employees to gain access to the south side of the dam from the north side.
