OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards:  a. On or about October 23, 2015, throughout the facility, employees operating forklifts were not wearing their seatbelts and were being exposed to crushing hazards.

29 CFR 1910.23(a)(5): Every pit and floor opening was not adequately guarded by standard railing or any other effected means:  a. On or about October 23, 2015, at the S-1 Slitter (Large pit area) employees looking down into the pit to visually check the height of the loop between the machines while the machine is running are being exposed to a 10 feet fall hazard.

29 CFR 1910.212(a)(3)(ii): The  point of operation of machines whose operation exposed an employee to injury, was not guarded:    a. On or about October 23, 2015, an employee working at the slitter was not protected from the point of operation while feeding in pieces of cardboard into the recoiler rollers.     Florida Metal Products Inc. dba FLAMCO was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 954283, citation number  01, item number 001 and was affirmed as a final order on March 17, 2014, with respect to a workplace located at 6940 Stuart Ave, Jacksonville, FL 32236.
