29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) Jobsite, 3245 Old Frankstown Road, Pittsburgh, PA 15239: Observed on or about November 05, 2015, an employee working at the bottom of an excavation that was approximately 10 feet deep with near vertical sidewalls was not protected from cave-ins. The employee was exposed to an engulfment hazard while engaged in work at the bottom of the excavation.
