Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about November 4, 2015, at the above addressed jobsite, employees performing carpentry work from foundation wall(s) and form work were exposed to falls, of nine (9') feet and ten (10") inches, created by the presence of unprotected sides and edges.

Each employee on ramps, runways, and other walkways shall be protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems. a. On or about November 4, 2015, at the above addressed jobsite, the ramp/walkway used to access the elevated formwork was not provided with fall protection, thereby exposing employees to fall hazards of nine (9') feet and ten (10") inches.

On or about November 4, 2015, at the above addressed jobsite, employees working on decking and concrete walls, were exposed to ends of rebar that were not guarded, thereby exposing employees to the hazards associated with impalement.
