29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 11/16/2015, the Brabender lab extruder needed a guard for the in-feed upper roll stack on the take off unit chill rolls. b) On or about 11/16/2015, the P-28 machine needed a blanking die guard. c) On or about 11/16/2015, the Sheet line OMV needed guarding for roll nips where there were gaps in the guards. d) On or about 11/16/2015, the CL-5 converting line green emboss roll guard was set 3/4" from the roll and needed adjustment to provide adequate guarding for nip. e) On or about 11/16/2015, on Converting line #2, the emboss roll section had an inadequate nip guard, the Flying knife roll needed guarding, and the empty core roll (Turret) nip needed guarding. f) On or about 11/17/2015, the Paper Machine #2 had an area guard missing the mid rail where a spray box used to be and the Reel end guard was missing part of the fence approximately 46" wide by 32" high.
