An employee left the powered industrial truck running and unattended with key inside the ignition, while the employee went to go and look for the supervisor inside the shop office, exposing surrounding employees to the danger of a struck-by of an unintended movement of the truck, on or about 11/18/15.

The employee is performing inadequate daily inspections of the forklifts.  There were deficiencies found with several Hyster forklift during the walk around inspection, which was not noted on the daily inspection check list, on or about November 18, 2015.       a.  Hyster #7188 - reverse, back-up and parking lights were inoperable. Also, oil leak was detected from this forklift.      b.  Hyster #7227 - horn is inoperable.      c.  Hyster #7184 - lights and horn are inoperable.      d.  Hyster #7142 - horn and reverse lights were inoperable.
