29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    (a)  Shultz Industries,  Inc at Eagle Landing Townhomes, Indian peaks Drive, Lafayette, CO 80026:  On and before November 18, 2015, the employer did not ensure that periodic inspections of the energy control procedures for the eXmark riding mower were conducted, and that adequate tools were present to remove the residual energy from the blades.  An employee used the wrong tool to clean out the mower, resulting in the partial amputation of one of the employee's fingers.  Condition resulted in employee exposure to an amputation hazard.

(a)  Shultz Industries,  Inc at Eagle Landing Townhomes, Indian peaks Drive, Lafayette, CO 80026; on and before November 18, 2015, the employer failed to ensure that periodic inspections of the energy control program had been performed and that the certification identified the various components of the energy control procedures as required.  Specifically, the periodic inspection of the energy control procedures for the eXmark riding mower failed to ensure that the adequate tools to remove residual energy from the blades were present.  Condition resulted in an employee utilizing the wrong tool and the partial amputation of one of the employees fingers.
